If an exemption is required for a clinical supervisor not employed at the same agency as the supervisee, a sample contract form is provided below which would meet requirements A.A.C. R4-6-212.01(1)(b)(i)(ii). This form may be used by the agency/clinical supervisor as a stand-alone document, however it is not required.

Supervision
- Employers and Supervisors can now utilize the forms above to submit the required verification forms for their supervisees. Forms may not be submitted directly from a supervisee unless they are in a signed, sealed envelope with an application.
